By signing this form, you consent to your images and videos being used for:

• Online and digital media – including our website, social media, email campaigns and newsletters.

• Printed materials – such as brochures, posters, banners and educational resources.

• Press and promotional activities – including fundraising campaigns, appeals and local or national media coverage.

• Images will be kept for a maximum of 5 years, unless otherwise required for historical records.
